What information do we collect?

For an appointment inquiry, we collect the contact details, appointment preferences and information you choose to provide. During care, your record may include medical and dental history, examinations, images, treatment notes, consent and billing or insurance information.

Please keep online inquiries and voicemail messages brief. Avoid sending detailed health histories, identification documents or payment-card details through ordinary email or an appointment note.

How is your information used and shared?

We use information to respond to you, arrange appointments, provide and coordinate dental care, maintain records and manage payment or insurance matters. Relevant information may be shared with authorized team members, service providers supporting these activities, and other care providers or insurers when consent or law permits.

Collection, use and disclosure are limited to the purposes explained to you, unless otherwise permitted or required by law.

What happens when you use online booking or AI voicemail?

North Arbor Dental uses Sikka for online appointment booking and offers AI voicemail for telephone messages. Information you submit through these services is processed to handle your inquiry and help our team follow up. AI voicemail may involve a recording, transcript or summary of your message.

You can ask our office about the service providers, storage location, retention and handling of your information before using these options. If you prefer human assistance, call our direct office line at (519) 896-7779.

AI voicemail supports communication; it does not replace a dental assessment. Separate, express and knowledgeable consent is required for using AI-generated outputs containing your health information for a secondary purpose such as training an AI tool.

What choices do you have?

You may ask how your information will be used, discuss communication preferences, or withdraw or limit consent where the law allows. Contact our office so we can explain any effect on care, communication or information sharing. Withdrawing consent does not undo earlier lawful use or override required record retention.

How do we protect and retain information?

Our information practices include administrative, physical and technical safeguards appropriate to the sensitivity of the information. Access is restricted to authorized purposes. We retain records for applicable legal and professional requirements, then arrange secure disposal when they are no longer required.

If a privacy incident affects your information, notification and reporting obligations under applicable law guide our response.

What about website data and cookies?

Our website and booking providers may process technical information, such as your IP address, browser details and cookies, to operate their services. Contact us for information about the tools used on the service you choose. External websites have their own information practices.

Can you access or correct your record?

Yes. You may request access to your personal health information and ask for corrections, subject to legal exceptions. Send a written request to our office, addressed to the person responsible for privacy. We will explain identity verification, applicable time limits and any permitted fees before processing your request.

Which privacy laws and resources apply?

Ontario’s Personal Health Information Protection Act (PHIPA) governs the handling of personal health information in our dental practice. Federal privacy law, including PIPEDA, may also apply to certain activities or information transfers.

This notice may be updated when our information practices change. The date above identifies the latest revision.
